  1. Head into the desired sub-account and select Sites > Surveys > Builder
  2. You can opt to start from scratch or pick a template. If you’re new to building surveys, I recommend choosing a template.
  3. Select the template and hit Continue. The survey will load up into the editing tool.
  1. Now, you can build out your survey by dragging and dropping the desired elements onto the page.
  2. Clicking on each element opens up the editing options on the right-hand side.

If the template isn’t quite to your liking and you want to customize it, add your logo in, etc., this is what you do:

  1. Click the icon in the top-right corner called Styles and Options.
  2. Now, you can change the design of your survey template by going through the different settings and making the necessary changes.
  3. Click to add a footer that will inform respondents of their progress.
  4. You can upload images to add as a background and page header.
  5. Pick from a wide range of pre-designed survey themes.
  6. In the Options section, you can choose what happens when respondents reach a certain point in the survey. For example, if they submit their results or are disqualified, you can opt to send them to a specific URL.

To add branching logic, you need to understand how to create and use custom survey fields.

Rather than relying on the pre-designed fields that GoHighLevel provides, you can use custom fields to gather specific information from your respondents.

  1. Head to the Custom Fields tab in the Survey Element menu and select Add.
  2. Choose the type of custom field that you want to add and click Next.
  3. Name the field (in other words, ask your survey question).
  4. If your question has multiple options (like in my example), add and name each option.
  5. When done, click Save.
  6. You can drag and drop your custom fields onto your survey pages as required.
Opportunities, or Business

When creating a custom field, you’ll notice a dropdown menu named Objects. From this menu, you can select Contact, Opportunities, or Business.

This determines where the information ends up after the respondent has completed the survey. 

Therefore, make sure you keep the object as Contact. This ensures that the answer to your question will appear on the contact card when the respondent’s details are added to GoHighLevel.

Now we get to the logic part!

To add branches to your survey, you have to create and add a custom field. There are only three types of custom fields you can use:

  • Dropdown (single)
  • Radio Select
  • Checkbox

If you choose Radio Select, do NOT check the Allow Custom Values option. If you do, you won’t be able to create logical branches.

Next, I highly recommend mapping out your survey and all its logical routes as a flowchart.

Frustratingly, GoHighLevel only presents the survey in linear form, making it confusing to determine which page a person jumps to after making a certain decision.

Once you understand the various paths of your survey, you can start creating the pages.

  1. Start with a blank survey page. Do NOT add anything other than one of your custom field questions. If you add anything else, the logical path won’t work.
  2. Drag and drop your survey question onto the page.
  3. Click to open the editing options.
  4. You’ll notice a Logic section is available.
  5. Each answer option allows you to select which page the respondent jumps to after selecting it. You can also opt to disqualify the respondent.
  6. Choose a page for each answer option until there is a complete path for every branch of the survey.

The survey builder’s automation is fairly limited. You can redirect someone to a specific URL, but that’s about the extent of it.

However, that’s not to say you can’t use automation on your surveys. You certainly can, but you have to use the workflow automation tool.

To do this, add the Survey Submitted option to your workflow. Then, you can create automated actions to decide what happens next.

You can also create different actions based on the survey answers by selecting the relevant filters.

For instance, if I want to add everyone who loves green widgets to a specific marketing campaign, I can select the question about widget colors in the Filters section of the workflow trigger and then select green.

To create a header image that perfectly fits on top of the form page, you must match the image’s background color to the page color. This will give you a seamless result that looks like one continuous design.
